Who should use this form?
Use this form if you would like to:
- share a new idea, innovation, or project opportunity
- identify a challenge that may benefit from collaborative problem-solving
- explore a partnership with the JP Centre
- connect with us about a concept that could improve services, systems, patient experience, research, or community well-being

What kinds of ideas can be submitted?
We welcome a wide range of submissions, including ideas related to:
- health system improvement
- patient and community-centred innovation
- research and evaluation opportunities
- service delivery challenges
- digital tools or new technologies
- collaborative projects and partnerships
- pilot opportunities and emerging concepts
You do not need to have a complete proposal. Early-stage ideas are welcome.
What happens after you submit?
Once your form is submitted, our team will review the information provided and determine the most appropriate next step. Depending on the nature of the submission, this may include:
- follow-up questions
- an invitation to an introductory discussion
- referral to a more appropriate partner or pathway
- internal review to assess alignment, feasibility, and next steps
Please note that submission of an idea does not guarantee that it will move forward, but every submission helps us better understand opportunities, needs, and areas for collaboration.
